(a) Eligibility. — A physician included in the registry is authorized to issue certifications to patients to use medical cannabis. To be eligible for inclusion in the registry: (1) A physician must apply for registration in the form and manner required by the bureau.

The bureau shall develop a standard certification form, which shall be available to practitioners upon request. The form shall be available electronically. The form shall include a statement that a false statement made by a practitioner is punishable under the applicable provisions of law.
Receipt of medical cannabis by a patient or caregiver from a dispensary may not exceed a 30-day supply of individual doses.
